Introdução: O câncer bucal corresponde a cerca de 30% de todos os tumores de cabeça e pescoço. Aproximadamente 90% dessas neoplasias malignas são carcinomas espinocelulares (CEC) e cerca de 15 mil casos novos são estimados a cada ano no Brasil. Objetivo: Avaliar os aspectos clínico-epidemiológicos de pacientes com CEC oral. Método: Estudo observacional, com delineamento transversal, quantitativo e retrospectivo, a partir da análise de prontuários de pacientes com diagnóstico histopatológico de CEC bucal atendidos em um centro de referência em Oncologia da Região Nordeste do Brasil. Variáveis clínicas e epidemiológicas foram coletadas e analisadas. A análise estatística foi realizada utilizando-se o teste de qui-quadrado (p≤0,05). Resultados: Entre os 298 prontuários avaliados, predominaram pacientes do sexo masculino (75,2%), com idade média de 60,4 anos, tabagistas e etilistas (62,0%). A queixa mais frequente foi a presença de ferida ou lesão na boca (61,1%), e o local mais comum foi a língua (62,1%). As variáveis clínicas revelaram estadiamento clínico avançado (III ou IV) em 76,4% dos pacientes. Entre os pacientes com idade até 50 anos, houve maior concentração de homens (p=0,015) e maior consumo de álcool do que entre os pacientes acima dos 50 anos (p=0,010). As demais variáveis não exibiram diferença estatística significante entre os grupos. Conclusão: As características clínico-epidemiológicas relacionadas ao CEC bucal devem ser consideradas para o planejamento de políticas públicas, a fim de prevenir novos casos e permitir a realização de diagnóstico precoce


Introduction: Oral cancer corresponds to about 30% of all head and neck tumors. Approximately 90% of these malignancies are squamous cell carcinomas (SCC) and about 15 thousand new cases are estimated each year in Brazil. Objective: Evaluate the clinical and epidemiological aspects of patients with oral SCC. Method: Observational cross-sectional, quantitative and retrospective design study, based on the analysis of medical records of patients with histopathological diagnosis of oral SCC treated at a reference center in oncology in Brazil's Northeast. Clinical and epidemiological variables were collected and analyzed. Statistical analysis was performed using the chi-square test (p≤0.05). Results: Among the 298 medical records evaluated, male patients predominated (75.2%), with an average age of 60.4 years, smokers and alcoholics (62.0%). The most reported complaint was the presence of a wound or injury in the mouth (61.1%) and the most common location was the tongue (62.1%). Clinical variables revealed advanced clinical staging (III or IV) in 76.4% of patients. Among patients aged up to 50 years, there was high concentration of men (p=0.015) and greater use of alcohol than among patients over 50 years (p=0.010). The other variables did not show a statistically significant difference between the groups. Conclusion: The clinical and epidemiological characteristics related to oral SCC should be considered when planning public policies, in order to prevent new cases as well as to allow for early diagnosis


Introducción: El cáncer oral corresponde a aproximadamente 30% de todos los tumores de cabeza y cuello. Aproximadamente 90% de estos tumores malignos son carcinomas de células escamosas (CCE) y se estiman alrededor de 15 mil casos nuevos cada año en Brasil. Objetivo: Evaluar los aspectos clínicos y epidemiológicos de los pacientes con CCE oral. Método: Estudio observacional, con diseño transversal, cuantitativo y retrospectivo, basado en el análisis de historias clínicas de pacientes con diagnóstico histopatológico de CCE oral tratados en un centro de referencia en Oncología del Noreste de Brasil. Se recogieron y analizaron variables clínicas y epidemiológicas. El análisis estadístico se realizó mediante la prueba de chi-cuadrado (p≤0,05). Resultados: Entre las 298 historias clínicas evaluadas, predominaron los pacientes del sexo masculino (75,2%), con una edad promedio de 60,4 años, fumadores y alcohólicos (62,0%). La queja más frecuente fue la presencia de herida o lesión en la boca (61,1%) y la ubicación más común fue la lengua (62,1%). Las variables clínicas revelaron estadificación clínica avanzada (III o IV) en 76,4% de los pacientes. Entre los pacientes de hasta 50 años, hubo una mayor concentración de hombres (p=0,015) y un mayor consumo de alcohol que entre los pacientes mayores de 50 años (p=0,010). Las otras variables no mostraron una diferencia estadísticamente significativa entre los grupos. Conclusión: Las características clínicas y epidemiológicas relacionadas con el CCE oral deben considerarse al planificar las políticas públicas, a fin de prevenir nuevos casos y permitir un diagnóstico precoz

BACKGROUND: We aimed to assess susceptibility pattern of Candida species isolated from horticulturists with onychomycosis to four antifungal drugs and to compare the effectiveness of conventional identification methods with matrix-assisted laser desorption/ionization mass spectrometry (MALDI-TOF MS). METHODS: This study was conducted in a community garden located in Teresina, State of Piauí, Brazil, in the year 2014. The samples were identified through phenotypic methods and per MALDI-TOF MS, being used PCR as definitive identification test. The susceptibility pattern to four antifungal drugs was determined according to Clinical and Laboratory Standards Institute (CLSI). RESULTS: Fourteen clinical isolates from seven different species were identified by the phenotypic method and by MALDI-TOF MS, with an observed concordance of 71.4% between the two methods. C. albicans (28.6%), C. parapsilosis (21.4%), C. guilliermondii and C. metapsilosis (both with 14.3%) were the most frequent species. With the exception of C. krusei, all species were sensitive to the tested antifungal. CONCLUSION: This is the first study of antifungal susceptibility of Candida in Piauí, Brazil. With the exception of C. krusei, no species showed resistance to the antifungal drugs used. This study suggests constants updates from the public databases used in MALDI-TOF MS to provide a rapid and accurate mycological diagnosis.

Objetivo: Avaliar a dor durante a marcação de lesões impalpáveis de mama com o uso do corante azul patente por meio de uma escala analógica de dor. Métodos: O estudo contou com 50 pacientes aten didas entre 2012 e 2015. Estas apresentavam lesões mamárias impalpáveis com diagnóstico de imagem feito por mamografia e ou ultrassonografia de mama. No dia da cirurgia, a marcação com o corante azul patente foi feita e, antes do procedimento, as pacientes foram orientadas a relatar a dor sentida após a ci rurgia através de uma escala visual de dor variando de 0 (nenhuma dor) a 10 (pior dor). Resultados: Das 50 pacientes estudadas, uma foi descartada por apresentar reações alérgicas. A idade média foi de 50,3 anos. A dor média durante a marcação foi de 5,13±2,84. Quando a marcação foi feita com ultrassono grafia, a média de dor foi 4,68±2,8, enquanto a mamografia apresentou 7,38±1,99 (p=0,02). Em relação ao câncer, observou se que sete pacientes com câncer apresentaram dor média de 5,43±3,2, enquanto as demais referiram uma dor média de 5,07±2,72 (p=0,769). Quando se analisa a dor em relação à idade, percebe se que a maior dor média é de 6,08±2,43 (p=0,465), relatada por pacientes acima de 60 anos. Conclusão: Pacientes que se submetem à marcação de lesões impalpáveis experimentam dor significa tiva, principalmente se a identificação for feita por meio de mamografia. Portanto, recomenda se, nesse grupo, utilizar procedimentos que reduzam o desconforto, como a utilização de anestésico tópico.


Objective: To evaluate pain during the marking of impalpable breast lesions with the use of patent blue dye through an analog pain scale. Methods: The study included 50 patients treated between 2012 and 2015. These had impalpable breast lesions with image diagnosis by mammography and ultrasound or breast. On the day of surgery, marking with the blue dye was made and, before the procedure, patients were instructed to report the perceived pain after surgery by visual pain scale ranging from 0 (no pain) to 10 (worst pain). Results: Of the 50 patients studied, 1 was dismissed by experience allergic reactions. The average age was 50.3 years. The average pain during the marking was 5.13±2.84. When the labeling was done with ultrasound, the average pain was 2.8±4.68, whereas mammography showed 7.38±1.99 (p=0.02). With respect to cancer, it was observed that 7 patients with cancer pain had a mean of 5.43±3.2, while the others reported an average pain of 5.07±2.72 (p=0.769). When analyzing the pain in relation to age, it is clear that pain is the highest average of 6.08±2.43 (p=0.465), reported by patients over 60 years. Conclusion: Patients who undergo marking nonpalpable lesions experience significant pain, especially if the identification is made by means of mammography. Therefore, it is recommended in this group use procedures that reduce discomfort, such as the use of topical anesthetic.

The aim of this study was to analyze fungal contamination on dental chairs at the clinic of a Higher Education Institution in TeresinaPI, Brazil, and to evaluate the effectiveness of different disinfectants: 70% alcohol and 1% sodium hypochlorite. We selected the five sites with most contact between patient and chair: headrest, backrest, armrests, seat and foot rest. Samples were collected from these sites on 14 chairs and inoculated in agar Sabouraud culture medium containing chloramphenicol. Pathogenic fungi were isolated from all sampling sites on the chairs. Highest frequencies were found on footrest, followed in decreasing order by seat, backrest, armrests and headrest. Fourteen species of filamentous fungi were identified, belonging to the genera Alternaria, Aspergillus, Cladosporium, Curvularia, Drechslera, Fusarium, Penicillium and Paecillomyces. After sampling, seven chairs were disinfected with 70% alcohol and seven with 1% sodium hypochlorite, and samples were taken again using the same procedure. No fungal growth was detected following disinfection with sodium hypochlorite, which was clearly more effective than alcohol, after which there was still fungal growth. This study highlights the need for the biosafety protocol to include cleaning and disinfection of dental chairs with 1% sodium hypochlorite after each attendance in order to prevent crossinfection.


O objetivo deste estudo foi analisar contaminação fúngica em cadeiras odontológicas na clínica de uma Instituição de Educação Superior em Teresina PI, Brasil, e avaliar a efetividade de diferentes desinfetantes: álcool 70% e hipoclorito de sódio 1%. Nós selecionamos os cinco locais com maior contato entre o paciente e a cadeira: encosto da cabeça, das costas, dos braços, assento e encosto dos pés. As amostras foram coletadas destes locais das 14 cadeiras e inoculadas em meio de cultura agarSabouraud contendo cloranfenicol. Fungos patogênicos foram isolados de todos os locais de amostragem das cadeiras. As frequências mais altas foram encontradas no encosto dos pés, seguido em ordem decrescente pelo assento, encosto das costas, dos braços e encosto da cabeça. Quatorze espécies de fungos filamentoso foram identificados, pertencente aos gêneros Alternaria, Aspergillus, Cladosporium, Curvularia, Drechslera, Fusarium, Penicillium e Paecillomyces. Após a coleta, sete cadeiras foram desinfetadas com álcool 70% e sete com hipoclorito de sódio 1%, e as amostras foram colhidas novamente usando o mesmo procedimento. Não foi detectado crescimento fúngico após desinfecção com hipoclorito de sódio, que foi claramente mais efetivo que o álcool, do qual ainda houve crescimento fúngico. Este estudo destaca a necessidade da inclusão no protocolo de biossegurança a limpeza e desinfecção das cadeiras odontológicas com o hipoclorito 1% após cada atendimento, a fim de prevenir infecções cruzadas.

Objetivo: avaliar o crescimento e o estado nutricional de crianças menores de cinco anos atendidas em uma clínica-escola e identificar fatores associados. Método: estudo descritivo, transversal, realizado com 353 crianças atendidas em clínica-escola. O crescimento e estado nutricional foram avaliados pelos índices estatura/idade, peso/idade, peso/estatura e IMC/idade. Dados socioeconômicos e de saúde foram coletados em questionários. Relações entre variáveis foram testadas pelo qui-quadrado e Correlação de Pearson. Este estudo foi aprovado pelo Comitê de Ética em Pesquisa, CAAE: 15722813.8.0000.5210. Resultados: baixa estatura foi encontrada em 6,8% das crianças. Excesso de peso estava presente em 3,6%, 15,2% e 15% dos meninos e em 4,6%, 13,7% e 14,8% das meninas segundo peso/idade, peso/estatura e IMC∕idade, respectivamente. Houve associação estatisticamente significativa entre estado nutricional e sexo, idade, frequência ao pré-natal e internação por pneumonia. Diarreia, pneumonia e doenças intercorrentes apresentaram correlação estatisticamente significativa com idade. Conclusão: a inadequação do crescimento e do estado nutricional aqui encontrada consistiu principalmente em elevada prevalência de obesidade, que estava associada a fatores biológicos e de saúde.(AU)


Objective: to evaluate the growth and nutritional status of children under five years old treated in a clinic-school and to identify associated factors. Method: descriptive, transversal study conducted with 353 children attending the clinic-school. The growth and nutritional status were evaluated by the indexes height/age, weight/age, weight/height and BMI/age. Socioeconomic and health data were collected from questionnaires. Relations between variables were tested by chi-square and Pearson´s correlation. This study was approved by the Research Ethics Committee, CAAE: 15722813.8.0000.5210. Results: short height was found in 6.8% of children. Overweight was present in 3.6%, 15.2% and 15% of boys and 4.6%, 13.7% and 14.8% of girls according to weight/age, weight/height and BMI/age respectively. There was a statistically significant association between nutritional status and gender, age, frequency of prenatal care and hospitalization for pneumonia. Diarrhea, pneumonia and frequent diseases showed statistically significant correlation with age. Conclusion: the inadequacy of growth and nutritional status found here consisted mainly of high prevalence of obesity, which was associated with biological factors and health.(AU)

Método: estudio descriptivo, transversal realizado con 353 niños atendidos en clínica-escuela. El crecimiento y estado nutricional fueron evaluados por los índices estatura/edad, peso/edad, peso/estatura e IMC/edad. Datos socioeconómicos y de salud fueron recogidos en cuestionarios. Relaciones entre variables fueron testadas por el Chi-cuadrado y Correlación de Pearson. Este estudio fue aprobado por el Comité de Ética en Investigación, CAAE: 15722813.8.0000.5210. Resultados: baja estatura fue encontrada en 6,8% de los niños. Exceso de peso estaba presente en 3,6%, 15,2% y 15% de los niños y en 4,6%, 13,7% y 14,8% de las niñas según peso/edad, peso/estatura e IMC∕edad, respectivamente. Hubo asociación estadísticamente significativa entre estado nutricional y sexo, edad, frecuencia al pre-natal e internación por neumonía. Diarrea, neumonía y enfermedades intercurrentes presentaron correlación estadísticamente significativa con edad. Conclusión: la inadecuación del crecimiento y del estado nutricional aquí encontrada consistió principalmente en elevada prevalencia de obesidad, que estaba asociada a factores biológicos y de salud.(AU)

UNLABELLED: The objective of this study was to evaluate the effects of botulinum toxin type A (BTX-A) on spastic foot in stroke patients in a rehabilitation program. METHOD: Hemiparetic stroke patients (n=21) enrolled in a rehabilitation program were divided into two groups. The first group (n=11) received a total of 300 UI BTX-A, and the second group (n=10) received 100 UI BTX-A. All patients were assessed at baseline and 2, 4, 8 and 12 weeks after injection for Modified Ashworth Score, time walking 10 meters, and the Functional Independence Measure (mFIM) motor score. RESULTS: The higher-dose group exhibited a significant improvement in spasticity, and both groups showed an improvement in time walking 10 meters and mFIM, with no significant differences between them. CONCLUSIONS: Our findings suggest that gains in gait velocity and functional independence were not correlated to BTX-A dose.

O objetivo deste estudo foi avaliar os efeitos da toxina botulínica tipo A (TXB-A) sobre a espasticidade de membro inferior em pacientes pós-AVE em reabilitação. Método: 21 pacientes hemiparéticos foram divididos em dois grupos que receberam doses de TXB-A de 300UI (Grupo 1) e 100UI (Grupo 2) e foram avaliados antes da injeção e 2, 4, 8 e 12 semanas após, quanto à escala de Ashworth modificada, tempo para andar 10 metros e escore motor da Medida de Independência Funcional (MIFm). Resultados: O grupo que utilizou dose mais alta teve melhora significativa da espasticidade. Ambos os grupos tiveram melhora do tempo para andar 10 metros e da MIFm sem diferença significativa entre eles. Conclusões: A melhora da velocidade de marcha e da independência funcional não foram correlacionadas com a dose de TXB-A na amostra analisada. .

Objetivo: Analisar a sobrevida em cinco anos dos pacientes atendidos por câncer de próstata no Hospital São Marcos. Métodos: Estudo descritivo epidemiológico, de base populacional, realizado em Teresina-PI, avaliando uma coorte hospitalar composta por 71 pacientes do Hospital São Marcos, inscritos no Registro Hospitalar de Câncer (RHC), de 2000 a 2001, com CID10 - C61. As variáveis estudadas na avaliação de sobrevida foram: faixa etária, estadiamento do tumor e cor da pele. Empregou-se o método de Kaplan-Meier no cálculo das funções de sobrevida em cinco anos e o teste de Kruskal-Wallis na comparação entre as variáveis. Resultados: A sobrevida específica por câncer de próstata foi de 78,5% em cinco anos. O risco de morte neste estudo aumentou com a faixa etária e o estadiamento avançado ao diagnóstico (faixa etária de 80 anos ou mais = 60%; e estádio IV = 63%). O teste de Kruskal-Wallis não mostrou variação estatisticamente significante entre os grupos. Conclusão: A idade e o estadiamento avançados ao diagnóstico diminuem a sobrevida dos pacientes.


Objective: To analyze the survival after five years among patients treated of prostate cancer at Hospital São Marcos. Methods: A descriptive population-based epidemiological study performed in Teresina-PI, evaluating a hospital cohort consisting of 71 patients of Hospital São Marcos, enrolled in Hospital Cancer Registry (HCR) from 2000 to 2001, under ICD10 -C61. The variables considered in the evaluation of survival were: age group, tumor staging and skin color. The Kaplan-Meier method was used in the calculation of survival functions in five years and the Kruskal-Wallis test in comparison between variables. Results: The specific survival rate for prostate cancer was of 78.5% in five years. The death risk in this study increased with age and advanced stage at diagnosis (aged 80 or above = 60%; and stage IV = 63%). The Kruskal-Wallis test showed no statistically significant variation between groups. Conclusion: The age and advanced stage at diagnosis decreased patients survival.


Objetivo: Analizar la supervivencia en cinco años de los pacientes atendidos por câncer de próstata en el Hospital São Marcos. Métodos: Estudio descriptivo epidemiológico, de base poblacional, realizado en Teresina-PI, evaluando una cohorte hospitalaria formada por 71 pacientes del Hospital São Marcos, inscritos en el Registro del Hospital de Cáncer (RHC), de 2000 a 2001, con CID10 - C61. Las variables estudiadas en la evaluación de La supervivencia fueron: la edad, el estadiamento del tumor y el color de la piel. Se utilizó El método de Kaplan-Meier para el cálculo de las funciones de supervivencia durante cinco años y la prueba de Kruskal-Wallis para la comparación de las variables. Resultados: La supervivencia específica del cáncer de próstata fue de un 78,5% en cinco anos. El riesgo de muerte en este estudio aumentó con la edad y el estadiamento avanzado del diagnóstico (edad de 80 años o más = 60%; y estadio IV = 63%). La prueba de Kruskal-Wallis no mostró variación estadísticamente significativa entre los grupos. Conclusión: La edad y el estadiamento avanzados en el diagnóstico disminuyen la supervivencia de los pacientes.

FUNDAMENTOS: Estudos demonstram que a realização da mamografia em programas de rastreamento diminui a mortalidade do câncer de mama. Entretanto, há indícios de grande desigualdade no acesso à realização deste exame no Brasil. OBJETIVOS: Analisar o percentual de mulheres que não realizaram mamografia segundo variáveis socioeconômicas e demográficas em mulheres de 40 a 69 anos de Teresina-PI. MÉTODOS: Estudo transversal realizado em 2010/2011, com amostragem multifásica em cinco etapas, realizando-se entrevista face a face com cada mulher sorteada. Processou-se os dados com programa SPSS 19.0, realizando-se análise uni e multivariada. RESULTADOS: Dentre as 433 mulheres que responderam o questionário, a taxa de realização de mamografia foi de 75,3%,sendo que, destas, 17,2% a haviam realizado há mais de dois anos. A não realização de mamografia esteve relacionada com raça parda/negra (p = 0,030), ausência de companheiro (p = 0,041), menor grau de instrução (p = 0,010), menor renda (p < 0,001), tabagismo (p = 0,006), não possuir plano ou seguro de saúde (p < 0,001). O Sistema Único de Saúde (SUS) foi responsável por realizar 56,3% das mamografias. CONCLUSÃO: 24,7% das mulheres teresinenses nunca haviam realizado mamografia e 37,9% não a haviam realizado nos últimos dois anos. A não realização deste exame foi associada à existência de desigualdades racial e socioeconômica.

This study focused on self-perceived oral health of the elderly and associated variables. The sample consisted of 321 functionally independent individuals 60 years or older living in Parnaíba, Piauí State, Brazil. Oral examinations were performed and questionnaires were applied during home visits. The elderly presented poor oral health, with mean DMFT 29.41 (SD = 4.10). However, their self-perceived oral health was positive, with 52% showing high scores according to the Geriatric Oral Health Assessment Index (GOHAI). Multivariate analysis showed that predictors of self-perceived oral health included the need for upper prostheses, oral mucosal lesions, and self-rated oral health. The most important predictor was self-rated oral health, with a weight of 20% variation in self-perceived oral health. In conclusion, subjective measurement of oral health in the elderly is less associated with their actual clinical status and more with other subjective factors.

Introdução: O câncer colorretal costuma ser pouco frequente em indivíduos abaixo dos 50 anos, persistindo ainda controvérsias quanto ao seu comportamento e prognóstico. Objetivo: Avaliar as características epidemiológicas, clínicas, histopatológicas e terapêuticas de pacientes com câncer colorretal, comparando os grupos de faixa etária até 50 anos e idade superior a 50 anos. Método: Trata-se de um estudo descritivo, em que foram analisados prontuários de 505 pacientes com câncer colorretal, atendidos de janeiro de 1998 a dezembro de 2008. Parâmetros clínicos e histopatológicos foram coletados. Resultados: Os indivíduos com idade até 50 anos corresponderam a 26,53 por cento da amostra e não houve predileção por gênero. Alteração no hábito intestinal como queixa inicial e anemia ao exame físico foram significativamente mais comuns no grupo mais jovem. Os pacientes até 50 anos exibiram também estadiamento linfonodal mais avançado, maior ocorrência de carcinoma espinocelular ou neuroendócrino e pior diferenciação histopatológica, bem como maior recorrência local, apesar de maior índice de tratamento cirúrgico e quimioterápico. Conclusão: O câncer colorretal em pacientes com idade até 50 anos mostrou-se mais agressivo e indiferenciado. As queixas, mesmo dos pacientes jovens, devem ser prontamente investigadas, a fim de possibilitar o diagnóstico precoce e maior chance de cura ao paciente com câncer colorretal.

Este estudo objetivou identificar a autopercepção da saúde bucal de idosos e investigar as variáveis associadas a essa autopercepção. Foram pesquisados 321 indivíduos, de 60 anos ou mais, funcionalmente independentes, residentes do Município de Parnaíba, Piauí, Brasil. Realizaram-se exames clínicos bucais e aplicaram-se formulários durante visitas domiciliares. Os idosos apresentaram precárias condições de saúde bucal, com média CPOD de 29,41, (DP = 4,10). A autopercepção da saúde bucal, medida pelo Geriatric Oral Health Assessment Index (GOHAI), no entanto, foi positiva, 52% dos idosos obtiveram escores altos no GOHAI. Resultados da análise multivariada demonstraram que os preditores da autopercepção foram necessidade de prótese superior, alterações em tecido mole e autoavaliação de saúde bucal. A preditora mais importante foi a autoavaliação de saúde bucal, com um peso de 20% na variação da autopercepção. Concluiu-se que a medida subjetiva relacionada à saúde bucal dos idosos está menos associada às situações clínicas que eles apresentam e mais ligada a outros fatores subjetivos.

INTRODUCTION: Leprosy is a potentially disabling infectious disease that evolves into emotional issues due to the prejudice that persists about the illness. The endemic has declined substantially with multidrug therapy (MDT) in the 80's; however, new demands associated with the reduction of stigma and the improvement of the affected people's quality of life have emerged. In Brazil, leprosy is still a public health problem. Piauí is the second state in the Northeast in prevalence and detection, and Teresina is a hyperendemic city. This study aimed to analyze the health-related quality of life (HRQoL) of people in treatment for leprosy in Teresina/PI. METHODS: An observational study was conducted using the SF-36 (a specific questionnaire for assessing quality of life), which sought the determinants of poor quality of life among people with leprosy, outlining the sociodemographic, clinical, and epidemiological characteristics of the 107 patients interviewed. RESULTS: The correlations between the variables showed five determinants of HRQoL: late diagnosis, multibacillary forms, reactions, disability diagnosis grade II, and prejudice. The profile of the participants showed that leprosy still affects the lower social classes in historically endemic areas, causing high percentages of secondary injuries that compromise the work capacity and quality of life of the affected people, perpetuating the stigma associated with the disease. CONCLUSIONS: The study reinforces the need to implement more effective strategies of disease control, due to the development of severe and disabling forms of leprosy is directly related to poor HRQoL in the same cured patient.

INTRODUÇÃO: A hanseníase é uma doença infecto-contagiosa, potencialmente incapacitante, que evolui com elevada carga emocional em função do preconceito que persiste sobre os acometidos. A endemia declinou substancialmente com a poliquimioterapia (PQT), na década de 80; entretanto, surgiram novas demandas associadas à redução do estigma e melhoria da qualidade de vida dos acometidos. No Brasil, a hanseníase ainda é um problema de saúde pública. O Piauí é o 2º estado do nordeste em prevalência e detecção, sendo que Teresina é hiperendêmica. Objetivou-se analisar a qualidade de vida relacionada à saúde (QVRS) de pessoas em tratamento da hanseníase em Teresina/PI. MÉTODOS: Estudo observacional realizado com a aplicação do SF-36 (questionário específico para avaliação da qualidade de vida) que buscou os determinantes da piora na qualidade de vida das pessoas com hanseníase, delineando o perfil sóciodemográfico, clínico e epidemiológico dos 107 pacientes entrevistados. RESULTADOS: A correlação entre as variáveis demonstrou cinco determinantes para QVRS: diagnóstico tardio, formas multibacilares, reações, grau II de incapacidade no diagnóstico e preconceito. O perfil dos participantes revelou que a hanseníase continua acometendo as classes sociais menos favorecidas em áreas historicamente endêmicas provocando percentuais elevados de lesões secundárias que comprometem a capacidade de trabalho e a qualidade de vida dos acometidos, perpetuando o estigma associado à doença. CONCLUSÕES: O estudo reforça a necessidade de implementação de estratégias mais efetivas de controle da doença, uma vez que o estabelecimento de formas graves e incapacitantes de hanseníase está diretamente relacionada à baixa QVRS mesmo no paciente curado.

CONTEXTO: A presença de sobrecarga em familiares cuidadores resultante do auxílio/cuidado prestado pode comprometer a saúde deles, implicar deterioração de relacionamentos e manutenção ou agravamento do quadro do enfermo. OBJETIVOS: Avaliar graus de sobrecarga, objetiva e subjetiva, de familiares de indivíduos com transtorno obsessivo-compulsivo (TOC) em amostras na rede pública e privada. MÉTODOS: Estudo descritivo-analítico constituído de: caracterização do sujeito com TOC (N = 30) e de seu familiar cuidador (N = 30), em amostras da rede pública (N = 30) e privada (N = 30), por meio de questionários sociodemográficos; confirmação diagnóstica (TOC) com uso do Mini International Neuropsychiatric Interview (MINI); aplicação da Escala de Avaliação da Sobrecarga de Familiares (FBIS-BR). RESULTADOS: Foram detectadas significância estatística entre as amostras na dimensão objetiva, com maiores graus de sobrecarga na amostra da rede pública, e significância na dimensão subjetiva apenas na questão relativa à "realização de tarefas de casa", com maiores graus na amostra da clínica privada. CONCLUSÃO: O estudo constatou graus de sobrecarga diferentes em familiares de sujeitos com TOC em tratamento, em amostras na rede pública e privada de assistência em psiquiatria.


BACKGROUND: The burden present in family members caretakers that results of the care given, could jeopardize their health, provoke the decay of relationships, as well as maintain or worsen the patient's condition. OBJECTIVES: Evaluate the levels of objective and subjective burden in family members of individuals with obsessive-compulsive disorder (OCD) in samples from the public and private network. METHODS: This is a descriptive-analytical study that characterizes the individual with OCD (N = 30) and their family caretakers (N = 30), in samples of public net (N = 30) and private net (N = 30), with use of standardized socio-demographic data questionnaire; diagnosis confirmation (OCD) by using Mini International Neuropsychiatric Interview (MINI); the Family Burden Interview Schedule (FBIS-BR) was applied. RESULTS: A statistical significance has been detected among the samples through an objective point of view, with higher levels of burden in the sample of public net; the significance through a subjective point of view was only perceived regarding "doing housework", with higher levels in the private practice. DISCUSSION: The study has verified different levels of burden in family members of individuals with OCD under treatment in samples from the public and private network of psychiatric assistance.

OBJETIVO: Determinar a prevalência e severidade de fluorose dentária em escolares com 12 anos, presentes na cidade de Teresina, Piauí. MÉTODO: Foram selecionados por sorteio, 374 escolares da rede municipal de ensino. Os exames clínicos foram realizados nas unidades escolares por um cirurgião-dentista calibrado (Kappa = 0,79) sob luz natural, com as crianças sentadas em carteiras escolares. Os dentes foram limpos, secos com gaze e em seguida aplicou-se o índice Thylstrup e Fejerskov para determinar a presença e/ou severidade de fluorose dentária. Os escolares foram questionados quanto ao número de escovações diárias, última visita ao dentista e satisfação em relação à aparência do sorriso. RESULTADOS: A prevalência de fluorose foi 61,5 por cento e não houve diferença entre gêneros (p> 0.05). A maioria das crianças apresentou TF = 1 (70,9 por cento) e os casos mais severos apresentaram TF = 4 (1,7 por cento). O dente menos afetado foi o primeiro molar e os mais afetados foram pré-molares, segundos molares e incisivos superiores. Apenas 5 por cento dos escolares nunca haviam visitado o cirurgião-dentista e 63 por cento o fizeram no ano anterior. Quanto à frequência de escovação, 30,3 por cento das crianças afirmaram escovar os dentes duas vezes ao dia e 57,5 por cento três vezes. A fluorose dentária, nos graus de severidade encontrados, não teve impacto na satisfação com a aparência do sorriso. CONCLUSÃO: A prevalência de fluorose foi alta entre os escolares da rede municipal de ensino, porém, com grau de severidade baixo, fato que sugere que a fluorose dentária não caracteriza um problema de saúde pública na amostra avaliada.


OBJECTIVE: Assessed the prevalence and severity of dental fluorosis in 12-year-olds attending public schools in Teresina, capital of Piaui, Brazil. METHODS: A sample of 374 adolescents was randomly selected from municipal schools. Clinical examinations were performed under natural light at school by a duly trained dental surgeon (Kappa = 0.79) with the children sitting at school desks. All teeth were cleaned and dried with gauze and scored according to the Thylstrup-Fejerskov (TF) fluorosis index. The students were asked how many times they brushed their teeth daily, the date of their last visit to the dentist and their degree of satisfaction with the appearance of their smile. RESULTS: The prevalence of fluorosis was 61.5 percent, with no difference between the genders (p> 0.05). The TF score of most children (70.9 percent) was 1 and the most severe cases (1.7 percent) had a TF score of 4. The least affected teeth were the first molars and the most affected were the premolars, second molars and upper incisors. Only 5 percent of the students had never seen a dentist; 63 percent had seen one in the last year. Almost one-third (30.3 percent) of the children reported brushing their teeth twice a day and 57.5 percent three times a day. The degrees of dental fluorosis found in this population had no impact on their satisfaction with the appearance of their smiles. CONCLUSION: The prevalence of fluorosis was high among schoolchildren attending municipal schools but its severity was low. This suggests that dental fluorosis is not a public health problem in this population.

Este é um estudo abrangendo idosos diabéticos participantes de um Programa de Diabetes em ambulatório de média complexidade do Sistema Único de Saúde com o objetivo de caracterizar os participantes segundo as variáveis estudadas. A investigação observacional, transversal, com 119 idosos, de ambos os sexos, foi conduzida entre setembro/2007-agosto/2008, aprovada pelo Comitê de Ética em Pesquisa da Universidade Federal do Piauí. Os dados foram tabulados pelo EPI-INFO versão 6.04d. Para a análise da associação entre variáveis, utilizou-se o teste do c2. Observou-se que 11,8% dos idosos moravam só, 33,6% eram diabéticos há mais de 15 anos. Predominaram as mulheres (69,7%), a faixa etária de 60-69 anos (58,0%) e o sobrepeso (60,5%). As proporções respectivas de risco elevado e muito elevado de doença cardiovascular (DCV) foram: 28,8% e 45,8%. O risco para o desenvolvimento de DCV apresentou associação com os valores do peso, índice de massa corporal, circunferência da cintura e sexo masculino.


This transverse, observational study of 119 elderly diabetics of both sexes enrolled in a diabetics program at a medium-complexity clinic of Brazil’s national health system was carried out from September 2007 to August 2008, with a view to characterizing the participants according to the variables studied. It was approved by the Research Ethics Committee of the Federal University of Piauí. Data were calculated using the EPI-INFO program, version 6.04d. The chi-square test was used to analyze the association between variables. It was found that 11.8% of the subjects lived alone; and 33.6% had been diabetic for more than 15 years. Most were women (69.7%), aged between 60 and 69 (58.0%) and overweight (60.5%). The respective rates of high and very high risk for cardiovascular disease (CVD) were 28.8% and 45.8%. The risk for CVD was associated with the values of weight, BMI, waist circumference and male sex.


Estudio con diabéticos idosos participantes de un Programa de Diabetis en un ambulatorio de mediana complejidad del Sistema Único de Salud, cuyo objetivo fue caracterizar los participantes según las variables estudiadas. La investigación observacional, transversal, con 119 ancianos, de ambos los sexos, fue conducida entre setiembre/2007- agosto/2008, aprobada por el Comité de Ética en Investigación de la Universidad Federal de Piauí - Brasil. Los datos fueron tabulados por el EPI-INFO versión 6.04d. Para el análisis de la asociación entre variables, se utilizó el test del c2. Fue observado que 11,8% de los ancianos vivían solos, 33,6% eran diabéticos hacía más de 15 años. Predominaron las mujeres (69,7%), la faja etaria de 60-69 años (58,0%) y el sobrepeso (60,5%). Las proporciones respectivas de riesgo elevado y muy elevado de enfermedad cardiovascular (ECV) fueron: 28,8% y 45,8%. El riesgo para el desarrollo de ECV presentó asociación con los valores del peso, índice de masa corpoal, circunferencia de la cintura y sexo masculino.

This study aims to evaluate the situation with breastfeeding in the State of Piauí, Brazil, at the time of the 2006 vaccination campaign. The sample consisted of 1,963 infants (< 1 year of age) from 45 municipalities. Medians and probabilities of breastfeeding categories were calculated by probit analysis. Multivariate logistic analysis was used to analyze the association between independent variables and partial and exclusive breastfeeding. The probabilities were: 16% for exclusive breastfeeding and 18% for predominant breastfeeding at 180 days of age and 58% for breastfeeding at 361 days. Median duration was 67 days for exclusive breastfeeding and 200 days for breastfeeding. The breastfeeding rate was higher for children whose mothers belonged to social classes C, D, and E (p = 0.024) and lived in the countryside (p = 0.009). Not nursing during the first 24 hours of life, using a pacifier, and bottle-feeding reduced the prevalence of breastfeeding (p < 0.001). The prevalence rates are below the recommended standards and reflect the need to prioritize actions to promote breastfeeding in the State.
